FOR CHILDREN WITH DIABETES

ADA Summer Camp Information
ADA SUMMER CAMP'S GOALS are to provide children who have diabetes with:
- A fun and safe camping experience.
- An emphasis on achieving good control of diabetes while adjusting to daily activities.
- An opportunity to develop confidence in their management of diabetes.
- Education about diabetes in an informal setting.
- American Diabetes Association Residential Summer Camp Program for Upstate New York
There will be two sessions offered this year for kids ages 8 to 16:
- Session 1: July 8-13, 2007 (ages 12-16)
- Session 2: July 15-20, 2007 (ages 8-12)
Camp is held on the Rotary Sunshine Campus in Rush, 15 miles from Rochester. The fee for either session is $500.
- Camp Rainbow Day Camp
Camp Rainbow Day Camp will be held August 13-16, 2007 from 9am to 3pm. Cost is $75. Camp is held at Hopkins Point Lodge in Mendon Ponds Park A.Application Process
To accommodate the maximum number of children and assure a positive camp experience, we must balance each cabin by age and gender. You should complete and mail the application and scholarship request, if applicable, with deposit by April 1, 2005. Early receipt of the application will better assure your child(ren's) acceptance.General lnformation
- Contact the ADA office at the address below to request an application form. An application deposit of $25 must accompany each application made payable to the American Diabetes Association.
- Any questions regarding the Residential Camp Program should be directed to your local American Diabetes Association Chapter or by calling the Rochester Chapter at 585-458-3040.
- Camper fee covers the total program including all medical supplies and food.
- Financial Assistance: Partial or full scholarships toward camp fees are available to children whose families qualify for financial assistance. The scholarship application is included with the brochure. You may also set up a payment plan with our office making the final payment by July 1, 2005. Camp fees can also be charged by using your Visa or Mastercard.
- Transportation to and from camp must be provided by parents/guardian.
- All meals and snacks are based on the ADA exchange list and all servings are weighed and/or measured. Exercise schedules are balanced with food intake and insulin levels. Blood glucose is tested a minimum of four times a day.
- All activities are supervised and meet the standards for safety set by the New York State Department of Health. The camp is licensed by the Monroe County Health Department and inspection reports are on file at its office on 111 Westfall Road, Rochester, 14692.
- New York State Health Laws require that each camper's immunization record be on file at the camp. Therefore, children will NOT be permitted entry into camp without a completed immunization form. You will receive a health history and exam form with camp acceptance. The parent/guardian must complete the health history portion and the child's doctor must complete the rest of the form. The parent/guardian is ultimately responsible for returning all completed forms to the ADA. All forms should be signed and dated where indicated.

Medical staff is carefully selected, trained and supervised to provide leadership and assure a healthy and safe environment. Medical residents, registered nurses, licensed practical nurses and registered dietitians provide daily supervision of management of diabetes and informal education about diabetes.CAMP ACTIVITIES
